Abstract
Workplace safety is a crucial aspect of the mining industry, which carries a high risk of work-related accidents. PT Kalimantan Prima Persada Site INDE, as a manpower service provider for PT Indexim Coalindo, faces significant challenges in maintaining contract sustainability due to suboptimal safety performance. This study focuses on analyzing and improving workplace safety through a Risk Analysis approach. Data were collected from workplace accident reports during the period of 2022–2024. Each incident was analyzed using the HIRADC (Hazard Identification, Risk Assessment, and Determining Control) method to detect potential hazards and assess risk levels. Further analysis was conducted using the 5W+1H technique and the Swiss Cheese Model to identify the root causes of accidents and design more effective preventive measures. The results of the study reveal several key contributing factors to incidents, including human error, inappropriate work methods, weak supervision, and organizational influences. This study offers actionable outcomes such as enhancing awareness of workplace safety, reinforcing the implementation of standard procedures and operational regulations, and encouraging the adoption of technological solutions within safety management practices.
