Abstract
Workplace safety is a top priority in the mining industry, where the high risks for workers demand effective strategies to prevent accidents. PT XYZ, as a manpower service provider for PT ABC in the mining sector, faces significant challenges related to contract sustainability and their safety performance. This research aims to analyze and improve workplace safety at PT XYZ. The study employs the DMAIC method, which stands for Define, Measure, Analyze, Improve, and Control. Data were collected from PT XYZ's accident records for the year 2022. These workplace accidents were then identified for their causative factors using the HIRA approach, which stands for Hazard Identification and Risk Assessment. From the HIRA data, analysis was conducted using a Fishbone Diagram to develop appropriate workplace safety programs to prevent future incidents. The results of this study are expected to provide a deeper understanding of the effectiveness of existing safety programs and identify areas needing improvement. The practical implications of these findings include raising safety awareness among workers, strengthening existing policies and procedures, and integrating technology and innovation into safety management. Overall, this research contributes to understanding how the implementation of HIRA can play a key role in creating a safe and sustainable work environment.
